The short answer: Most side sleepers with back pain do well at 5–6 out of 10, lighter people a touch softer. The Helix Midnight Luxe is our first pick, with zoned lumbar coils under a soft Euro top; the Brooklyn Bedding Signature Hybrid (Medium) is the better value, the Casper Original Hybrid adds zoning for lower back pain, the Purple Restore Hybrid (Soft) is the premium choice, and the Novilla Bliss is the budget one. Put a pillow between your knees whatever you buy.

How a side sleeper's spine should look
Picture a straight line from the middle of the back of your head down to your tailbone while you lie on your side. On a good bed, that line stays roughly straight. On a hard bed, the shoulder and hip hold you up like two posts and the waist hangs between them; on a very soft bed, the hip sinks furthest and the line bends down in the middle. Both bend the spine sideways, and both can show up as back pain in the morning.
That is why side sleepers need a little more softness on top than back sleepers, and why the support underneath matters just as much. A soft foam bed with a weak base sags; a firm bed with a thin top bruises the shoulder.
Helix Midnight Luxe (Queen)
The bed many other back-pain lists rank first, now sold on Amazon: a 6-out-of-10 hybrid with a zoned lumbar section and a soft Euro top.
Why it is here: Helix aims it squarely at side sleepers: a 6-out-of-10 feel, three foam layers for the shoulder and hip, and zoned lumbar coils so the waist stays level. Many other side-sleeper lists rank it first; now it is on Amazon from the maker's own storefront.

Purple Restore Hybrid (Soft) (Queen)
Purple's grid on a coil base, in the softer version that side sleepers with shoulder pain tend to ask about.
Why it is here: The softer Restore pairs Purple's grid with coils: the shoulder drops into the grid, the coils hold the middle. Expensive, and stock was limited when we checked.

Three side-sleeper habits worth changing
- Use a pillow between the knees. It keeps the top leg level and stops the pelvis rotating forward. A firm foam knee pillow is better than a bed pillow folded in half.
- Check your pillow height. Your head pillow should fill the gap between your ear and the mattress. Too flat and the neck bends down; too tall and it bends up.
- Do not curl tightly. A gentle bend at the hips and knees is fine; pulling the knees to the chest rounds the lower back for hours.
A knee pillow costs a fraction of a mattress and is the habit most worth starting tonight. Ours are compared in the knee pillow guide.

Heavier side sleepers
If you weigh over about 230 lb, go one step firmer and choose coils. The heavier-sleeper picks are built for that. Lighter side sleepers (under about 130 lb) can go a step softer; the upper back pain picks are a good place to look.
Before you order: reader questions
What firmness should a side sleeper with back pain choose?
Around 5–6 out of 10 for most people, a little softer if you are light, a little firmer and coil-based if you are heavier.
Is memory foam or hybrid better for side sleepers with back pain?
Both work. Foam contours closely at the shoulder; hybrids keep the waist supported and sleep cooler. Hybrids usually suit heavier side sleepers better.
Why does my lower back hurt when I sleep on my side?
Often because the waist sags between shoulder and hip, or the pelvis rotates forward because the top leg falls. A supportive bed and a pillow between the knees address both.
Is a firm mattress bad for side sleepers?
For most side sleepers, yes. It pushes back hardest at the shoulder and hip, which can cause pressure pain and bend the spine.
Neutral Spine is not a medical provider. A mattress can make sleep more comfortable; it does not treat a spinal condition. Pain that follows an injury, spreads down a leg, or comes with numbness, weakness or fever needs a doctor first. Full disclaimer.
